What can you expect from Choice Home Care Specialists:
- Care Plus and Respite. We tailor our protocol to meet the
clinical and non-clinical needs of each client/patient.
- Companionship. We strive to reduce significant pressure
and stress on the family and help each family make the most of time spent
together by being sensitive to our client’s emotional needs.
- Health Watch. Under the supervision of our LPN Manager we
focus on medical best practices.
- Homemaker Services. In addition to personal care, we
offer meal preparation, light housekeeping, and errand and shopping
services.
- Hygiene Programs. One-to-one attention for
all personal care needs such as bathing, toileting and grooming.
- Progress Reports. We help monitor and
manage physical symptoms and needs and provide the doctor and family with
important information to oversee the client’s care.
- Personal Care Plans. A Licensed Practical
Nurse (LPN) will assess all new clients to help determine a plan of care. At
the initial visit we will establish a Personal Client Care Plan which
outlines daily services.
